A Good Faith Estimate:
Shows the expected charges for the care reasonably expected at the time the estimate is created.
Is not a bill or a contract.
Does not require you to receive the listed services.
May change if your needs or recommended plan of care change.

Initial Evaluation: $199
Follow-Up Visit: $175
Targeted Dry Needling: $125
The number and type of visits you may need depend on your individual condition, goals, and progress. Modern Athletic Co. will provide a written, patient-specific estimate when required or when requested.
If your final bill from Modern Athletic Co. is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill through the federal patient-provider dispute resolution process. You must generally begin this process within 120 calendar days of receiving the bill.

Good Faith Estimates.
Modern Athletic Co. is a private-pay physical therapy practice. We do not bill insurance companies directly. Payment is due from the patient at the time of service.
Private-pay care allows you and your physical therapist to make decisions based on your needs and goals. Visits are fully one-on-one, treatment is flexible, and pricing is clear.
Modern Athletic Co. may be considered an out-of-network provider by your insurance plan. Some commercial insurance plans may reimburse part of the cost of medically necessary physical therapy. Coverage and reimbursement are not guaranteed.
Before treatment, you are responsible for asking your insurance company about:
Out-of-network physical therapy benefits
Deductibles and reimbursement rates
Referral or prior authorization requirements
Claim-filing deadlines
Visit limits and covered services
You are responsible for paying Modern Athletic Co. in full, submitting any claims, tracking those claims, and communicating with your insurance company. Any reimbursement is determined by your insurance plan.
Medicare and Medicaid have different billing rules. Please contact Modern Athletic Co. before scheduling if you are covered by Medicare, Medicaid, or another government health program.
Private Pay and Out-of-Network Insurance.
Superbills.
Modern Athletic Co. may provide a superbill for skilled physical therapy services. You can submit this document to your insurance company to request out-of-network reimbursement.
A superbill lists:
The care you received
The date of your visit
Diagnosis and service codes
The amount charged and paid
Provider and practice information
A superbill does not guarantee reimbursement. Your insurance company decides what it will cover and pay.
You are responsible for checking your benefits, submitting your claim, tracking it, and appealing a denial if you choose.
Modern Athletic Co. can correct clerical errors but cannot change accurate records, codes, or charges to influence reimbursement.
Maintenance memberships, wellness services, personal training, yoga, and other non-skilled services are not billed as physical therapy. You may receive a payment receipt for these services, but they will not appear on a physical therapy superbill.
HSA and FSA Payments
Modern Athletic Co. accepts HSA and FSA cards when available. Physical therapy may be an eligible medical expense, but eligibility depends on your plan and the care provided.
We cannot guarantee that your HSA or FSA administrator will approve a charge. You are responsible for checking your plan’s rules and keeping any receipts or documents it requires.
Personal training, yoga, wellness services, and other non-medical services may not qualify as eligible expenses.
Payment & Cancellations
Payment is due at the time of service unless other arrangements have been made in advance.
Please provide at least 24 hours’ notice if you need to cancel or change an appointment. Cancellations made with less than 24 hours’ notice and missed appointments may be charged a %50 of the scheduled service fee to the card on file.
Modern Athletic Co. may waive this fee when appropriate due to an emergency or other unexpected circumstance.
